1. As people age, the brain naturally changes effecting memory, learning and other cognitive functions. These cognitive abilities effect how well we do every day tasks and whether we live independently.
2. We provide assessment and care planning of patients with memory loss like dementia including Alzheimer’s disease, at any stage of impairment.
3. Cognitive assessment includes a detailed history, physical exam, review of patients reports and records.
4. We do functional assessment of basic and instrumental activities of daily living.
5. Reconcile and review high risk medications and evaluate neuropsychiatric and behavior symptoms including agitation, depression and anxiety. We address safety evaluation, advance care planning and any palliative care needs.
